When was Delta first detected in Egypt?


	The delta variant is officially in Egypt with the MOH making an official statement. It is a bit unclear when exactly was the first case detected, but since we are not actively doing gene sequencing, it is hard to tell how many cases are here or how long have the delta infection been going around.


	Are we in the 4th wave and is Delta responsible?


	We are definitely in the 4th wave, this is also announced by the government, cases have been on the rise on the daily official reports and also in clinical practice. Whether delta is responsible or the lack of precautions and normal socialization is the cause, it’s hard to tell, again we are not testing specifically for it, but most probably it is the combination of both.


	What is Delta and Delta plus?


	The delta variant, was first detected last year in India, where it has been ravaging the nation and has since spread to many other countries.


	Delta has several lineages with slightly different sets of mutations. One of those is the delta — B.1.617.2 — which is also now the dominant coronavirus variant in the United Kingdom and the United States.


	The variant has mutated to result in another variant, known as delta plus, that has been reported in Britain, India and the United States. The effect of delta plus remains to be studied thoroughly,


	Overall, health experts describe delta as the most “fit” variant of the coronavirus and the most capable of transmission and breakthrough infections.


	Why is the delta variant a concern?


	Early research suggests the delta variant is about 50 percent more contagious than the alpha variant which was first identified in the United Kingdom and became the dominant variant there and in the United States. Alpha was already about 50 percent more transmissible than the original variant of the coronavirus first identified in Wuhan, China, in 2019


	The delta variant has changed the fight against the coronavirus as data has proven that it is highly contagious, and is likely to cause more severe disease, there is also evidence that breakthrough cases (vaccinated cases) may be as transmissible as unvaccinated but the extent is still unclear.


	Data also suggests that vaccinated, asymptomatic individuals can still transmit it. It is also suggested that the vaccines prevent more than 90 percent of severe infections but may be less effective against milder infections.


	In terms of hospitalization, the verdict is still not out on this one,Public Health England (PHE) found that the variant may be associated with a higher risk of hospitalization, and some early research from Scotland states that the risk of hospitalization with covid-19 from the delta variant is about twice the risk from the alpha variant — with unvaccinated individuals at the greatest risk.


	Delta and Children?


	Delta is known to easily infect children and since only the Pfizer is approved for children from 12. The rise in recent pediatric infections is strongly attributed to delta variant.


	How effective are vaccines?


	A study published in July in the New England Journal of Medicine found that the Pfizer-BioNTech vaccine was 36 percent effective against symptomatic disease caused by the delta variant after the first shot of the two-shot regimen, but 88 percent effective after the second shot. There is also Evidence that the Astrazenaca vaccine efficacy takes longer to drop


	In terms of the exact percentage and whether or not a drop in effectiveness of vaccines happen and if so when, remains to be decided. The only vaccine cleared for a booster dose so far is Pfizer. More data will be released in the coming period, however, one thing all experts agree on, is that vaccination in general is much more protective than no vaccination at all, and the hospitalization prevention seems to be applicable to all vaccines.


	How to protect yourself?


	Maintain social precautions, you need to be even more strict that last couple of years and please get your Vaccine.

We have all been hearing and watching all the media on how some vaccines are stopped, resumed, stopped again and more and more issues.....etc. it has been confusing and anxiety inducting to many patients, so we thought it would be a good idea to explain what is happening in the simplest and straightforward way without complex numbers and statitstics, this is our opinion as a practice team


	 Here is what we know so far:

<ul>
	<li>It all started with the AstraZeneca vaccine with the reporting of some cases developing thrombotic events (جلطات) in some in Europe after the vaccine roll out there. After a lot of back and forth between the “no evidence” group and the “cases are evidence” group, it was confirmed that there is an association between the (Vector) AstraZeneca vaccine and the occurrence of thrombo-embolism, after which the vaccine was paused for a little bit.</li>
</ul>
<ul>
	<li>It was then decided to resume the vaccination rollout in those countries who paused it (France, Spain, etc..) because statistically the risk of thrombosis and fatality from covid systemic disease is way higher than that of the vaccine</li>
</ul>
<ul>
	<li>More countries started noticing this event but the data showed that albeit a very rare event, it is more common in younger individuals, risk increases for those below 55</li>
</ul>
<ul>
	<li>More countries started restricting its use for individuals younger than 55, and the UK restricted it for individuals younger than 30</li>
</ul>
<ul>
	<li>The European Union is still freaking out about it and have decided to switch their vaccines to Pfizer and Moderna</li>
</ul>
<ul>
	<li>Egypt will not stop AstraZeneca but is also providing Sinopharm but it is not as commonly found</li>
</ul>
<ul>
	<li>Same issue occurred with the other vector vaccine J&amp;J</li>
</ul>
<ul>
	<li>Recent reports suggested association of thromboembolism with Moderna as well but more data is needed</li>
</ul>

	 What Revive’s Consultants and Medical team think about all this?

<ul>
	<li>The thromboembolic events is not a de novo occurrence, it does occur with some medications despite being very rare. A famous drug for this is heparin which is a drug ironically used to dissolve blood clots, this condition is referred to as Heparin induced Thrombocytopenia, it was noticed that the vaccine induced thrombosis is very similar to that</li>
</ul>
<ul>
	<li>The benefit of the vaccine still outweighs the risks but we always advice our patients who are below 55 to take Sinopharm. If they want to or can only get AstraZeneca then we always recommend a blood thinner and do a coagulation text in a week (this is our guideline at revive, this is not an international guideline recommended) but we always individualize recommendations according to science and patients. And so we apply the science principle that being on the vaccine puts you at a high risk of thrombosis especially in a certain age group, under which we would offer a blood thinner. We also check for rise in D dimer early to ensure that, in the event of hyper-coagulability we catch it early (THIS IS ONLY UNDER YOUR DOCTORS GUIDANCE)</li>
</ul>
<ul>
	<li>We will most probably need an annual vaccine, this is what most scientists say considering how the antibodies do not rise to a satisfactory level in all individuals, and when they do they last from 9-12 months tops. So even if the vaccine choices and data are limited now, they will be better next year.</li>
</ul>

	<li>Last but not least please take any vaccine you can put your hands on, benefits always outweighs the risks.</li>

	1- Which vaccine should i take? 


	Whatever is available! while they all come with different efficacy and effectiveness, they all help and work towards the global goal of preventing deaths and decreasing spread of infection.


	2- Can we trust the Chinese vaccine?


	Chinese vaccine is known to have lower efficacy (79%-86%) in comparison to the rest, however, this is still an excellent number, also no serious side effects were reported. The method of which the vaccine is created is very conventional and old school, in which its made of a dead virus part that stimulates the production of antibodies in your system, also the UAE has been an early adopter to this vaccine including the government officials and no serious side effects have been reported, perhaps the only disadvantage is lower efficacy than the rest of the vaccines, but also lower side effects reports.


	3- Which vaccines are available in EGYPT? 


	Currently the oxford Astrazeneca vaccine and the chinese Sinopharm


	4- What if i want to choose another vaccine?


	You have to wait or travel to take the vaccine privately, we should also mention most governments distribute it through an organized scheme, so it is not really available privately except maybe the UAE.


	5- Can we trust Astrazeneca (oxford) vaccine after the pause of its administration in some European countries? 


	Yes you can! One, because there is no clear evidence that the vaccine is the cause for the thromboembolic incidents and Two because its only 40 cases with thromboembolic events from millions who took it in the UK and Europe and Three, the WHO has released a consensus about continuing the vaccination and that benefits outweigh the risks way more.


	6- What are the lethal side effects of covid vaccine?


	No documented critical side effects except anaphylaxis which is a life threatening allergy to the vaccine, those have been reported mainly with the mRNA vaccines which we do not have available in Egypt, and also its treated with an instant epi-pen. The rest of the documented side effects are the expected side effects of any vaccine, they are not lethal and medical professionals know how to deal with it.


	7- What if I am allergic to vaccines?


	If you have documented allergies to vaccine, you will be advised on precautions before taking it and monitored while and after you are take it.


	8- If Vaccines are not 100% effective why should i take it?


	Because 100% is not the Goal! the goal is to decrease serious disease and deaths rates, this is usually the goal with many vaccines, its very rare that you get a vaccine that is 100% effective, but the idea is to try to avoid, serious disease requiring hospitalization and deaths and a plus would be to prevent symptomatic disease. an example would be the difference between Astrazeneca vaccine, while it doesn’t prevent disease with a high 95% as pfizer and moderna does, it has a very impressive effect in preventing serious disease and deaths with 100% efficacy after the second dose


	9- Would the Pfizer and Moderna vaccine mess up with my genetics?


	It is a new Technology so we do not have all data, but it is highly unlikely.


	10- Is the pandemic a government conspiracy backed up by big pharma?


	No its not! no one can manufacture a pandemic.


	11- Are vaccines safe for children?


	We do not know! clinical trials were always done on adults but Moderna is starting clinical trials on children


	12- Are Vaccines safe for the elderly? 


	Yes


	13- Do we have to take it yearly? 


	We do not know yet, papers on antibodies levels and timeline are still in the making


	14- Does it protect from variants? 


	Too early to say but some vaccines should more efficacy than others on variant albeit being very humble effect., data on this is still very fluid so again more time is needed for a sure answer
